The health care sector is experiencing major changes due to extensive research and development activities. As nurses play a crucial role in the delivery of health care, they need to embrace new and innovative techniques to provide effective and best possible treatment to their patients. Several institutional and personal barriers can hinder nurses from adopting EBP (Wood, 2013).

The two potential barriers that can prevent continuation of my evidence based practice change proposal in the future is poor coordination among the clinical team and staff turnover. My project focuses on Coronary Artery Bypass Graft patients and working with these high-risk patients need the best coordination and team work from the whole clinical team including devising appropriate discharge planning. Poor communication between the clinical team members can affect the outcome. My strategy is to recommend three interventions: staff appreciation events, monthly review of the continued adherence to the changes made, and monitoring the staff compliance by the administration. These interventions can motivate the clinical team to continue to pursue the change proposal. The second barrier I assume is staff turnover. There is lot of staff turnover in our hospital. Floating nurses and agency nurses on contract can affect the continuation of my change project. My strategy is to promote ongoing training which is absolutely necessary. These training needs to be scheduled and to be listed in the calendar every week.

Hospital management can help to overcome these barriers. The nurses preferred to consult their supervisors and colleagues to obtain needed information. Hospital management can create a conducive environment and provide socializing opportunities for nurses to promote peer-to-peer information and knowledge sharing (Melnyk & Overholt, 2011).

A preterm a baby is a baby born before 37 weeks of pregnancy. According to the Maternal and Child Health Bureau a study done in 2013 preterm babies born to different races in the United States “11.54 percent of babies were born preterm. Of those baby’s 16.53 percent of babies were born to non-Hispanic black women, 10.29 percent of babies were born to non-Hispanic white women, 10.15 percent of the babies born preterm were born to Asian/Pacific islander women, and 11.58 babies born preterm were born to Hispanic women.” (Child Health USA 2013, 2013) Racial and cultural disparities in preterm are sometimes attributed to education, age, income, demographics, if the mother has access to proper care and insurance, health behaviors such as abuse, and preexisting health conditions.

It is fortunate that with modern technological advances preterm babies have a better advantage in surviving then in the past. But unfortunately, there are also risk factors that are involved of preterm birth. According to the Centers of Disease Control and Prevention babies that are born before 32 weeks are at a higher risk of death and disabilities which include breathing problems, feeding difficulties, cerebral palsy, developmental delay, vision problems, and hearing problems. (Reproductive Health, 2017) Extreme low birth weight babies and preterm babies pose an economical cost to the family, health care industry, and the community, as more care will cost especially if the baby needs to be at the healthcare facility for a longer period for assessment and growth. Long term disabilities and long-term care can affect the families economically and socially at home as they may need ongoing professional help and will also lead to stress and problems for the family. Families need to prepare themselves when a preterm baby is born and be ready to address and complications, disabilities, and difficulties.

Potential barriers

Potential barriers to my EBP project continuing desired results include resistance from staff and waning commitment from staff (Pexton, n.d.). Early mobility of ICU patients involves a lot of work from nurses such as additional assessment and physical demands of mobilizing patients, so I could see how after the initial push the results may decrease. To overcome this, education could be ongoing along with encouragement of staff to express their concerns. Allowing staff to communicate regarding the implementation may bring up points that were not thought of or planned out, leading to improved processes. Waning commitment is another barrier that may reduce the desired outcome of early mobility in the ICU. Many nurses, including myself, feel that we are always given more tasks and asked to do more with less. This may lead to reduced compliance as the program continues. This barrier may be overcome by implementing a communication plan that reaches all ICU staff members and focuses on the positives and “wins” of the project (Pexton, n.d.). Positive feedback from me regarding increased positive patient outcomes due to early mobility would keep the staff motivated and hopefully continue the desired results. For EBP practices to continue, management also needs to facilitate a culture of improvement and support. Managers and leaders can do this by supporting communication, encouraging staff participations, and establishing priorities (HCPro, 2008). Through education, communication, and management support my EBP project of early mobility of ICU patients could show continued positive results.
