The purpose of the assignment is to give students a chance to make connections between the ideas in the various units in the first half of the course. When preparing your paper you should draw on at least two of the Unit presentations
(Units 1 through Unit 4 inclusive) and three of the required/optional readings from any of the first four units. Use only the course materials for your essay.

You may draw on personal experience and observation, but the course materials must always take precedence. Before beginning your paper, make sure that you read over both the instructions for this assignment (above) and the “Expectations” for the assignment set out below.

Drawing on the material in the first four units of this course, analyze two and no more than three of the medical technologies and medical
practices, past and present, that have and still can threaten women’s
health and well being. For example you could consider doctor’s attitudes and the general beliefs about women that permeated society,
the impact of poverty on health, some of the current forms of birth control, inadequate drug research, medicalization, antipathy towards lesbians, reproductive technologies, and so on. (These are only

suggestions; you can choose your own topics). Conclude your short paper with suggests about ways that these issues can be mitigated.

1.Resources to Use in Preparing Mid-Term Paper:Draw on at least
two of the units and at leas t three of the readings in Units 1–4 (required or optional). You are to use only the course materials
for this assignment.

2.Theoretical Perspective: When doing your analysis you are expected to base your findings in one or more theoretical perspectives that you have dealt with in the first four units of the course.

3.Length:The essay should be no more than800wordsor about three to four(12 point) typed double-spaced pages in length plus the bibliography and cover page. A paper much shorter than this would not cover the topic adequately. I calculate a page to be 25 typed lines of 10 words each making a total of 250 words per page.

4.Cover Page: Your paper should have a cover page which would clearly set out the following: your name, the course number, the professor’s name, type of assignment and any other information you think is important.

5.Reference Notes:You must have approximately two reference notes per page. You should follow proper academic form. If you do not have a reference guide to follow, use the referencing in the course Manual as a model. Style sheets and reference manuals are available on the internet–simply type “MLA” or “APA” “style sheet” into Google and you will find a number of sites from which to choose. When using one of
the required readings cite the author of the article as a reference, e.g. (Murray, et al.98). In the bibliography use the author’s name, title of the article and the publication data such as the journal title the article is from or the title of the book along with the date of publication. For an example of a bibliography see the bibliography at the end of the course or at the end of any of the required articles.

Scenario

John, a health management student completing an internship at Memorial Hospital, has been appointed chair of a multidisciplinary clinical taskforce by the hospital’s CEO. The taskforce will design a new operational system to reduce the waiting time of patients entering the hospital’s emergency room (ER). Although John had no clinical experience, he had successfully completed a course in operations management prior to beginning his internship and was excited to apply his new knowledge for solving a “real” problem for the hospital.

The hospital CEO told John that when a patient entered the hospital’s ER, it could take up to eight hours from the time the patient was initially triaged by a nurse to the time the patient was either discharged home or admitted as an inpatient by the physician. The CEO said, “Due to quality of patient care issues, this timeframe is unacceptable and the taskforce needs to come up with solutions to this problem. My goal is to reduce the “turnaround” time for the patient from eight hours to two hours.”

Prior to being assigned as the chair of this taskforce, John had informally observed the operations of the hospital’s ER and noted that many of the bottlenecks causing patient care delays were caused by operational issues such as nurses filling out duplicate forms and a lack of communication between the hospital departments (for example, radiology) when the ER physicians ordered tests or were waiting for test results to confirm their diagnoses. These bottlenecks caused a slow turnover of the ER’s examination rooms and unnecessary paperwork resulting in the ineffective use of both the physicians’ and nurses’ time.

In addition to John, the CEO assigned Dr. Smith, the medical director of the hospital’s ER, and Mary, the ER nurse manager, to the taskforce. As chair of the taskforce, John scheduled an initial meeting for 10:00 a.m., the following Monday. John was surprised that both Dr. Smith and Mary arrived twenty minutes late to the meeting saying that this was “taking valuable time away from their normal assignments.” John started the meeting by first introducing himself. Before this meeting, he had no interactions with Dr. Smith and Mary. He then reviewed the current statistics of the average wait time for a patient presenting to the ER and the hospital’s CEO desire to reduce this time. He then opened the meeting for comments and suggestions.

Dr. Smith spoke first, “In my opinion, the current operational systems that we have in place are just fine. We just need more ER physicians and examination rooms so that more patients can be seen.” Dr. Smith told John to recommend that the operational systems were good enough and that the hospital should build a new wing for additional ER exam rooms and hire more physicians.

Interrupting Dr. Smith, John said, “The hospital has a very limited capital budget and no funds have been allocated for building more facilities. We need to redesign the operational system to be more efficient and effective.” Dr. Smith gave John a stern look and reminded him that he, not John, was the medical director and therefore, knew what is needed and what is best for the hospital’s ER.

Mini Lecture
The IMIA-NI (Nursing Informatics: Special Interest Group of International Medical Informatics Association)definition, agreed at their General Assembly in Stockholm in 1997, and amended for clarity at the General Assembly in Seoul, 1998 defined Nursing Informatics as the integration of nursing, its information, and information management with information processing and communication technology, to support the health of people world-wide.

Evolution

The term “medical informatics” has its foundation with the French term “informatique” which concerns all the aspects of the computer as a tool for use in processing of information. The term “medical informatics” was used before “nursing informatics” and “health informatics”. Medical informatics is the use of computers for classification and retrieval of data and for the management health care information, enabling use to explore and better understand the informational and cognitive foundations of medicine3 (Blois, 1986).

In 1985 Hannah defined nursing informatics as the use of information technology by nurses carrying out their duties and which are in relation in relation to any function in the purview of nursing (as cited in Ball et al, 2000).

Graves and Corcoran (1989) state that nursing informatics is a “combination of computer science, information science, and nursing designed to assist in the management and processing of nursing data, information, and knowledge to support the practice of nursing and the delivery of nursing care.”

Romano (as cited in Ball et al., 2000) adds a further dimension to this definition by asserting that nursing informatics is “the application of the principles of information science and theory to the study, scientific analysis, and management of nursing information for the purpose of establishing a body of nursing knowledge”.

The ANA definition of nursing informatics. Nursing informatics is a specialty that integrates nursing science, computer science, and information science to manage and communicate data, information, knowledge and wisdom in nursing practice.(ANA, 2008) Nursing informatics facilitates the integration of data, information, and knowledge to support patients, nurses, and other providers in their decision-making all roles and settings (ANA, 2008 p. 1). Aiding in their roles is what makes this definition role-oriented. Role oriented definitions seem to be the most complete of these NI definitions. They describe how the NI specialty functions under the broader scope of health informatics yet has specific components such as: differentiated practice, defined research, organizational representation, educational development, and credentialing (2008). As the definition has evolved, it has become more specific to nursing.
